The AAIB report on the recent Ahmedabad Indian Airlines crash said that the cause of the plane’s crash after takeoff was that both engines were stuck in the fuel switch. It turned out that this was entirely the fault of the pilots.

The fuel switches changed from run to cut off.. Within seconds, the engines stopped fuel. At 1:39 am, the pilot made a Mayday call. Air traffic control responded.. But there was no response from the pilot, the AAIB report said.
